About the Event

Any unplanned undesirable event, be it a breakdown, production delay, missed delivery, near miss, quality issue, accident or incident, is an opportunity to learn about what went wrong. Without an understanding of these events, history will repeat itself.

An RCA seeks to identify deep and underlying root causes, which can be expected to link many issues that would otherwise appear to be unconnected. It is from this connectedness that RCAs draw their true power. When one root cause is fixed, it does not just prevent the original problem from re-occurring, it is addressing a common root cause of many problems which will now neither re-occur nor perhaps even occur at all.

This course will take delegates through some principles of RCA and incident investigation processes, highlight pitfalls and challenges likely to be encountered in implementing effective RCA processes, and then provide a methodology for undertaking effective RCAs. This methodology will include how to get to true root causes without getting stuck at the superficial “human error” level; it will include models and theories that allow an investigation to dig deep into underlying, latent or systemic root causes, so addressing both the problem at hand and potential future incidents – moving RCAs from being a reactive investigation tool to a pro-active improvement methodology.

Our trainer has a quarter century of experience in implementing RCA processes and undertaking investigations, much within the process industries, so you can benefit from his experience.
